Park Hotel Tokyo at Shiodome, Tokyo is a really good place to go according to my friends (couple) that went there not too long ago. Here is the website: https://www.ozmall.co.jp/hotel/0030/facility.aspx
They have restaurants, cafe, bar, and a relaxation center (aroma therapy). You can see the pics on the website mentioned above to see the atmosphere of each place.
Spa is a really famous place to take a girlfriend, since all the girls like beauty tips and massages and aroma therapy and what not performed professionally, I promise you she will feel more beautiful when she leaves Spa. There are many in Tokyo that does couple Spa, so you and her can both get beautied up at the same time:)

You're going to Tokyo to see your girlfriend, so it is safe to assume that she is currently visiting there or actually living there.
Since she is there and you are not, it is very difficult to say what places will surprise her, since it is her city that she lives in and she knows it better.
In reality it should be the opposite, and she would probably know the places of beauty that might impress you!
I suggest you take a day trip OUT of Tokyo with her. To say nearby Hakone or Nikko are popular destinations that are close to Tokyo.

If you want a romantic evening, I would recommend the New York Bar in the Hyatt Hotel in Shinjuku. It's a bit pricey but the views are breath taking and there's a live band playing jazz. If you liked Lost in Translation then it's definitely the right place to go!

I agree with the above answer. Chicks are suckers for posh hotels, and Japanese luxury hotels don't mind non-guests checking them out.
