I have a good base in music theory and have practiced and practiced improvising but still don't sound that great. Any advise will be greatly appreciated.

Concentrate on the left hand. Play chords at first then let your right hand work from those chords. Its not going to make you a GOOD improviser- that just comes with practice - but if you learn to keep the chord structures contained and flowing you cant actually go wrong.
The really tricky bit is making the jump from just noodling around to sounding like Art Tatum.
I'll let you know how to do that when I've worked it out.

Do you have any fake books? Those could be useful for practice. Are you studying with a teacher who could help you?
Also, just curious, what style are you looking at? If it's jazz, you could take some classical pieces and practice playing them in a jazz style, using improv, but going off the cords already written.

If you haven't seen it, take a look at Fazil Say's jazz interpretation of Mozart's Rondo Alla Turca on You Tube. Amazing!!
