Usually it's just an intuition and if you think he needs to go, don't hesitate to have him checked out!

If you are uneasy about something, you should always trust your gut. But also remember that you don't have to rush him in...a call to the doctor could be reassuring and they'll let you know whether you should come in based on your conversation about his symptoms. Doctors are used to this and you won't bother them. That being said, there are plenty of "common sense" answers that I'm sure you're well aware of about when to rush to the doctor or ER - unresponsiveness, VERY high fever, strange behavior or excessive crying following a fall or bump...
There's no need to stress when the doctor is only a phone call away.

If you think he needs to go then go. Better safe than sorry. I usually call the doctor or the nurse and tell them the symptoms and they can usually say whether to bring them in or not.
Trust your gut and don't ever think that you are silly for taking your baby to the doctor.

Is he/she running a temperature? Coughing/congested beyond the usual so that it prohibits them from breathing well? Are they sleeping more than usual? There are a million ways to tell, but if you think something is wrong then you should trust your intuition and get it checked out. At least call the dr.
