Do you measure it somehow, or what? How?

is there some way to avoid this measuring?

the doctor or widwife measures it by inserting two fingers inside the vagina and checking how open the cervix it is. Usually one finger is one centimeter so if they can feel an opening to fit 5 fingers you are about 5cm dilated. They don't actually put two hands up there to check for 10cm, they just use two fingers and flip them over.
Sounds gross and uncomfortable, but is quickly done.

EDIT: no, they have to check you to see what your progress is so they know when you can/can't have pain medications - if you are already 8cm they won't give an epidural anymore because it probably won't have time to take effect, or if you are only 3 cm they may send you back home till you progress further. Or it can tell them if the labour is stalling and may need assitance like the drip to speed things up or maybe needing a c-section. In short, it has to be done but is very quickly done and not really painful or invasive. Tell your midwife about any hesitations you have about it.

The midwife measures you with their fingers.

insrt fingers,pill out&measure(by the time you'r dilated enough for baby)see if you can grt three in&pull out&roughly measure.Takes a while to be@the right size.(6-10cm,i ?)

a nurse or midwife or a doctor does that for you.
