In our family, if the person was an uncle or aunt, we were taught from the beginning to call them that. The only exception that I know of was my mother and her nephew and my mom was 9 months younger than her nephew. Any child was required to call an adult aunt/uncle, mr./mrs/miss, or cousin.

Add'l.: Doesn't matter in public, still the same answer; but in the workplace, if you both work together, it could be a problem called nepotism. If nepotism isn't a problem and you don't mind co-workers knowing the family relationship, then the title should be used.

If you are a sibling it is always suitable. My husband is his father's youngest child and his half-bros/sis' have children older than him. THEY do not call him Uncle at all because it's kind of strange but the younger ones do.

My family didn't practice the whole Uncle/Aunt we just call them by their first names, however my friends' children call me Aunt Ashley and there is no relation there whatsoever. It really just goes w/ how you or they want it.

whether someone is called uncle/aunty is not decided by how old he/she is, but from the time they has a nephew or niece, and those allowed to call them uncle/aunty is the nephew or niece (sons/daughters of their brothers/sisters)
