All immigrants had to have a medical and language exam at Ellis Island.

As far as I know Ellis Island was primarily for people planning to emigrate permanently. And most of the first class passengers would have been wealthy people, more likely to be visiting the US instead of moving there. So fewer of them would have been put through all that screening. Anyone settling in the US would have been tested, but not visitors.
