I always use a chlorine bleach mixture. Buy a spray bottle. Put gloves on and ventilate the room because the vapors are bad for you. Put about a 50/50 mix of bleach and water. Spray it on all of the mildewed areas. Leave the room. Come back 15 minutes later and get the places that you missed. Leave the room again. Come back and rinse well.
If you want to remove hard water deposits, just use a spray on cleaner like 409. However, use it on a different day than you use bleach. Bleach should not be mixed with other cleaners.

Try Mr. Clean magic eraser.
That thing works for everything. I've used it on the grout in my shower, and it was amazing. It has even taken off hair dye that had splashed on my wall, when nothing else even faded it.

after using the magic sponge eraser you can buy paint that is used just for the grout to refresh it and make it like new again

Tilex bathroom spray and a skinny wire brush. Then, good old fashioned elbow grease! Just don't let the Tilex get on your skin and ventilate the area well.

I spoke to the tile installer where I work and he said vinegar is the best whitener but be sure to rinse it off after.
