I have just today got a link to a planner that can find long distance bike routes.
It is in Dutch though, so maybe not your first choice.

Bereken route = calculate route.
When you point at a part of the route it changes colour to red and the name in the planned route will be underlined, often you can click on the name for more information.
Plaats routepunt = add waypoint.
Kaart = map.

For Germany use the ADFC maps. They cost 6.90 EUR each, but cover a relatively large area and are detailed enough.

For certain cycle routes there are specialised guides available, like the Danube or Elbe cycleways.Look into them too.
