I have a 4'3 inch daughter, Size 4.5 boot. We have a 130 board. 8.75 inches wide. Is this board too thin for a size 5 boot?
How should the bindings be put on? Straight or at an angle?

As a rule of thumb, usually between the chin and nose, but kids can get away with a little longer board and grow into it. My youngest is a little taller and rides a 135, but he is an pretty advanced rider, so this board may be a little long. The other factor that you don't mention is weight, that is the important factor for sizing. Many boards will list weight ranges.

The bindings should be 12 - 9 degrees (toe forward) in the front and 6 - 3 degrees (toe forward) in the back. More angle will make learning easier, but will make switch riding harder to learn later on. That board will be wide enough, it is OK if the boot hangs over just a little, thats why the bindings are "lifted" and the underside of the boots are beveled heel and toe.

Front foot is usually the one in front when you catch yourself from falling forward, front foot when you slide accross a floor in your socks or the hand on the bottom when you cross your arms.

A 130 should be fine. As far as angles on the bindings, that's up to her preference and at what angle she is most comfortable with. Has she snowboarded before? If anything, go to a ski/board shop and have a person in the boarding section help you and her.
