Recognition of the fact that they do not belong together is the first step; follow that with tactile reinforcement, perhaps writing out that the two do not belong together, making a rhyme to memorize that reinforces the same, etc.

As your brain undertakes the tactile process, the very action of it, gradually your mind shall become disciplined to the two being no longer linked, and it shall be habit.

Jack is right, you begin by realizing that they do not belong together, you could separate them by rote ( incantation) by telling yourself over and over they do not belong together, or you could do some pictographs putting a big "X" through the picture of them together...

Well you could start hurting yourself everytime you see a cheese grater. Basically associate a bad feeling with the thing you want to unlink from.
