With this information, we can only guess. You do not say what kind of turtle, how big it is, or what kinds of cares it is getting.
My guess is that it is showing what is called an ear infection, and probably some Respiratory Infections as well. This is common in turtles that are being kept in conditions that are less than good for them- which is easy because there is so much misinformation about them.
To have healthy, happy turtles, they want three things:
1. Good water- a LARGE habitat with warm water (75-80F) that is kept very clean with a good filter.
2. Good sun- a safe, easy to climb on basking site that is warmed to about 90F and lit nicely in a way that illuminates the rest of the tank- while allowing shady hiding spots. Using a UVB bulb is helpful.
3. Good food- a mix of 1/2 good pellets, and 1/2 live or frozen/thawed 'fish foods' like small fish, shrimp, worms, insects, etc.
Now- ear abscesses USUALLY do not go away without good antibiotics from a vet. You can try either 'Turtle Sulfa Dip' or the fish medicine 'Stress Coat' per directions (but not both, please.) They can help with some of the medical problems involved.
