I searched for such a book recently, with UK English, and found the Oxford A-Z of Grammar and Punctuation by John Seely. I find it is clear and concise.

This book is a handy pocket size, inexpensive and could be what you are looking for, unless you want something more in depth.

Hands down, the best, in my opinion is Strunk and White's The Elements of Style. It's a slim volume that wastes no words and gets to essentials. One of the authors is E.B. White who wrote Charlotte's Web and Stuart Little. Perhaps that is why such a concise book about grammar and punctuation also manages to be charming.
There is a link below to Amazon to a new illustrated hardback edition, where you can read the table of contents and excerpts. You can no doubt pick up an older paperback at a used book store. The used paperbacks on Amazon were going for as little as 37 cents.

I find the following two reference books a must for writers:

1. The Elements of Style by Strunk and White.

2. The Writer's Digest Grammar Desk Reference by Gary Lutz and Diane Stevenson.

I know this sounds silly, but the best grammar book with the most straightforward grammar rules in is the Ladybird book.

Please don't think I am taking the mickey. I have studied English Language at University level and the lecturers said that the book I have mentioned covers most of the basics in the simplest and most straightforward manner possible. There are other books you could get, that would cost a great deal more money, and probably tell you the same things in a much more complicated manner.
However, if you don't wish to be seen with a children's book, "Rediscover Grammar with David Crystal" was also recommended by the lecturers. This is still in a very simple format.
