There are a couple of large malls in San Antonio...
- Rolling Oaks Mall - 6909 N. Loop 1604 E @ Nacogdoches
- The Shops at LaCantera - 15900 LaCantera Pkwy
- The Forum - 8200 Agora Pkwy
- Northstar Mall - 7400 San Pedro Ave
- Ingram Park Mall - 6301 NW Loop 410
- Rivercenter Mall - 820 E. Commerce (Downtown)
- The Quarry Market - Basse Rd
Rolling Oaks, Northstar Mall, Rivercenter, and Ingram Park Mall are San Antonio's indoor malls. The Forum, Quarry and LaCantera are outdoor malls.
As for the biggest...
- Rolling Oaks mall has 4 anchor stores (Sears, Macy's, JCPenney, and Dillards)
- Ingram Park Mall has 4 anchor stores (Dillard's, Macy's, JCPenney, Sears)
- Rivercenter Mall has 2 anchor stores (Dillards and Macys)
- The Shops at LaCantera have 4 anchor stores (Neiman Marcus, Nordstrom, Macy's, and Dillard's)
- Northstar mall has 4 anchor stores (Dillards, JCPenney, Macy's, and Sacks Fifth Avenue)
- The Forum has Best Buy, Old Navy, Borders, Target...just to name a few...it's the biggest outdoor shopping center in the city.
- The Quarry has Borders, Movie Theatre, Old Navy, Gap, Bed Bath and Beyond, Office Max, and Whole Foods
I'd say the biggest malls are Rolling Oaks, Northstar, LaCantera, Forum, and Ingram.
A really huge mall 30 miles NE of San Antonio (I-35) is the Prime Outlets at San Marcos outlet center with a ton of stores.

You have had many answers and several very good opinions. However, you simply asked for the name of the largest mall and the location.
Northstar Mall is the largest having over 200 vendors is about 50 stores larger than the next largest mall in the city. It is located at: 7400 San Pedro Ave.
The other malls are all fine and can meet your needs dependent on where in the city you are located.
NOTE: By the way it is great to see Lalana back!
EDIT: If a mall is any accumulation of stores which you can drive between in your car, as ffperki suggests, then the whole north side could be considered a shopping mall.
There is a difference between a mall and a shopping center where multiple stores have individual entrances to the outside. At the outside, a mall should be an enclosed area thus including the Shops at La Canterra. The Quarry, The Rim, The Forum and areas like those, while very popular and useful are shopping centers.
The mall area of Ingram Park Mall is 150 stores. What I want when I go to a mall is the ability to walk between stores in inclimate, cold or very hot weather in comfort, such as when doing holiday shopping. Otherwise I usually avoid malls and go directly to the desired store at a shopping center.

Kevin gives you quite a venue there. I would agree that the Forum is the biggest and best outdoor for the money. La Cantera is a more expensive breed of mall outdoor mall. Northstar is the most popular indoor mall, but for value and fun as far as indoor, I would put my money on Rivercenter mall where you can go to the comedy club, the imax, the other smaller theaters, shop for a beautiful cocktail gown at a much more reasonable price at dillards or macy's and still eat lunch at the food court or watch the spurs at hooters. You can even go outside near the river on a pretty day and listen to the music of the central americas live. This is one cool Mall.

I don't consider the strip mall ones malls.
I would only consider North Star, Rolling Oaks, River City, and Ingram malls.
Rolling Oaks is an older mall and more on the small side. It is quiet and a great place to take younger kids. I like it because you can avoid the crowds there.
North Star is probably the biggest and it is very traditional in the mall sense. It is sort of the upscale one in SA.
River City is also very large. It is a real spectacle as there is so much to see and the architecture there is interesting. You can eat outside on tables by the riverwalk and it has lots of touristy stores. If I were visiting from out of town I would choose it because one would get a feeling of seeing something different not just every mall-every city usa.

North Star? It may be bigger but I am not sure of that it really depends on how you look at it. I think that Ingram Square and area is larger. Please note that I added and area. As in indoor or closed mall it would be North Star but if you count all the stores from the over all indoor and out door malls that make up Ingram then it would be Ingram. I count a mall as all of the stores that share the same parking lot and as it is now you can drive in the parking lot over half a mile

Old "tried and true" Northstar Mall.
I've always wondered why it didn't try to merge with Central Park mall... set up a shuttle service, and become a (cue echoing wrestling announcer's voice) MEGA MALL!
