This is in Harry Potter and the Deathly Hallows when Harry is talking to Dumbledore right after he is unsure whether or not he is truly dead.

It's the piece of Voldemort's soul that resided in Harry. When one makes a horcrux they make their soul unstable. So the more hocruxes you make, the more unstable your soul becomes.

By the time Voldemort went to kill the baby Harry, he'd already made five horcruxes and planned to make his sixth and final horcrux with Harry's death. However, as we all know now, Voldemort's killing curse rebounded on himself. Because of the five horcruxes he'd already created he couldn't die. But he could loose physical form. When that happened, unbeknown to him a piece of his soul broke off and sought out the only living being in the destruction. Harry thus became the unknown sixth horcrux, giving Voldemort the seven pieces of soul he desired but did not know he already had when he made Nagini what he believed to be the sixth horcrux.

In order to defeat Voldemort once and for all, Harry had to destroy all the horcruxes. And that meant allowing himself to die. For by dieing he took the piece of Voldemort's soul inside of him into death. And with it no longer attached to him, he could easily return to the mortal world in order to face Voldemort one final time.

It is what became of Voldemort after he refused Harry's offer to allow him to apologize, and turn away from the darkness. Harry ended up 'killing him' so to speak, and that was all that was left.

It is Voldemort, otherwise know as Tom Riddle, in his true form of what he will bear in death.

(In otherwords, the shriveled up lump on the floor is what Voldemort will become after he dies)

my theory is that is whats left of valdermorts soul, it was never really explained though
