I live in California what is the form to fill out on the EDD for maternity leave?

anybody knows?? please help thanks...

Your employer and/or your doctor will give you the forms. Both gave them to me, but I used the one my employer gave me since it had a lot of information filled out already.

Your employer will have the form just ask.
