It does look fairly convincing, however, unless his finger is several inches longer than normal and disjointed it can't be possible. He'd have to be missing the inside of his face like his sinus passages and facial muscles.
You would also see a large bulge if he could get his finger under the skin of his face. If you look at the part of his finger at the nostril then you will notice it is just the tip of his knuckle that is placed tightly against his nostril. The angle of the finger going it is also wrong. The finger tip is just a prop that's been wedged in the corner of his eye. Or, it is simply an altered photo which makes the most sense to me.

Yea it's real, i used to do that but i can't be bothered to anymore.
