If the hard drive is being used such as copying and deleting, the file or whole hard drive may get corrupted. You wll then have to reformat your hard drive.

Most of time, there will be nothing happen to your hard drive. If you pulled it out in the middle of the read / write process, it may have high risk to damage the harddisk and your data may be gone.
For such case, it is still not the end of the day and you still have some chance to get your data back. Try to use those data recovery program to help.
If you harddisk is not functioning anymore, i.e. no spinning sound at all. You may consider to send it to the harddisk manufacturer for exchange (if it is still under warranty period) and they can still retreive the data for you (but you've to pay some fee for data recovery).
Wish the above can provide any assistance to you.

Depends on what you were doing. Playing music? nothing. Coping a file over to it, or defrag.. you probably lost data. As long as it isnt spinning, you can remove it all you want. You do not have to "Stop" the hardware to remove it. I pull my wireless out and my usb drives all the time.

you always want to safely remove it otherwise your external hard-drive/flash-drive will become corrupted most people don't worry about it cause they think nothing will happen but really it something will happen later once your drive is corrupted you won't be able to send or transfer files from/to the device.

in must cases nothing will be happened , but if you pulled it out in time of data access (read/write) it will be a high risk of permanent damage .

I did that too and learnt my lesson, corruption happened, not as serious as head crash but entire NTFS file system is gone.
