Also, how are you notified when benifits stop, phone call, letter...?

every individuals disability benefits end at different times. Depending on Your disability, it may be permanent....for life...it may be for a couple of years.
Usually your disability office contacts you via letter sent to your home to have a meeting with the benefits officer to see if you're still eligable or if you're benefits will be expired.

Go to your gov't site for disablity and read how things work but basically that's how it works

For commercial insurance, you have to provide evidence of continuing disability. unusally this is nothing more than a form to be completed by your doctor or a periodic basis.
They would notify you when they are no longer satisfied that you are disabled.
For permanent disability, they might just send you a form periodically, and give you a short but reasonable time to have it completed and returned.
If litigation is involved, the ins co might demand you be examined by the physician of their choice. If their physician finds you to be not disabled, benefits would stop.

Just about all determinations to stop benefits would be in letter form. Then, there is a record of it.
