You have to ask your employer if you can take a leave of absent before you take it. If they say no, then you can try getting a note from your doctor saying that your not well and need some time off. You would be eligible to collect sick benefits through unemploment, as long as you have a doctors note to prove it and I think they give you 15 paid weeks or more for sick leave.

Nope... If you take 2 weeks of from work without approval you voluntarily quit. BTW no unemployment benefits either.

No. You can't collect unemployment unless you have been laid off.

And, you can't take a "personal leave of absence" without express permission of your supervisors.

No, you basically quit your job. Unemployment only kicks in when you loose your job, not when you quit.
