Could be 'The Testament of Gideon Mack' by James Robertson - but how long ago did you read it? This was published pretty recently (in paperback, anyway).

Gideon slips into a river gorge and gets swept away into underground caverns, where he meets the Devil (or perhaps he hallucinates it all?). He's 'missing presumed dead' for at least 3 days. His life wasn't going brilliantly before, but it gets far worse when he tries to tell people about his experiences. The problem is, he's a Church of Scotland minister. You'd think that if someone can confirm that the Devil exists, that automatically proves that God exists... but people just don't want to hear. I don't remember any doppelgangers, though.

I can thoroughly recommend it anyway, even if it isn't the one you're looking for.
