You have diarrhea when your period starts due to hormones in your body called prostaglandins. These hormones cause the uterine contractions that expel menstrual blood, and also activate involuntary muscle contractions in your bowels causing diarrhea. My doctor told me that I can take ibuprofen 3 or 4 times daily starting 4 days before my period. The ibuprofen reduces the effects of the prostaglandins and should prevent or lessen your diarrhea at the onset of your period, but you do have to remember to start taking it a few days before your period.
I also sometimes get uterine pain with bowel movements during my period. It's just because the uterus is so close to your intestines and colon. It is fairly common, but if you keep taking the ibuprofen for pain, it should help. You can take up to 800mg of ibuprofen for pain every four hours, so if it is really bad that is the recommended prescription strength dose.

Diarrhea is very common for most women when they have their period and aches usually comes with it because the lining of your uterus is being pulled off and squeezed through an opening the size of a pea. It affects your back the most, and the more athletic you are, the less you feel the pain. If you're not athletic, then it's going to hurt. Midol, Pamprin helps the bloating, cramping, and some of the moodiness. Just expect and be prepared for it when it comes.

I've read that during your menstrual cycle, the body releases hormones that trigger contractions of the abdominal muscles (this is why we get cramps). These hormones also sometimes affect the intestines, and as a side effect, causes diarrhea. Unfortunate, huh?

Most likely you have a tilted uterus. Getting diarrhea when you get your period is a common side effect or a tilted uterus (I was surprised to find that out). About 10% of women have a tilted uterus, so it's pretty common. As far as aches, I don't know.

The hormones that cause your uterus to contract and expel blood from your body during your period are not organ sensitive. In other words, those hormones affect other organs in your body, too. Since your intestines are in the same general area as your uterus, they are made to contract as well. This causes diarrhea, gas, etc. Lovely, eh?

I had a similar problem, when I had my period I would have IBS symptoms. Mine was aggrevated by stress and a poor diet.
When I made sure I was properly hydrated and eating lots of fruits and vegetables and avoiding junk food, the problem went away.
