Depends how exactly it was broken. If for example you stepped on it, then you are going to have to replace it at your cost. If something internal stopped working due to normal usage, then the provider generally covers it. Make sure to contact your provider and see if they can remotely diagnose the box. It could be a simple configuration error that they can fix by sending a signal remotely or they may just have you do a factory reset.

Well, the best answer I will give you is try plugging your Xbox360 into the modem, aslong as the cables fits everything should be good. This is assuming your 'broadband modem box thingy' is a router/modem. If not, it could cause problems with your PC and Xbox fighting over available IPs. If that is the case you can try and get another IP from your Provider or shutting off the PC or go to your local computer shop and pick up a router for about £15-20. You may find the link below helpful for setting up a network.

You will most likely have to pay for a new one, unless your current modem/router (you didn't specify which) is under warranty, in which case contact the manufacturer.

it depends on the service provider, A year ago my provider operated by the rule "if it stops working on its own, they'd replace it, but if the engineer comes round and nothing was wrong with it, they charge you"
