Porn movies are shot over several days. The actors take drugs and stimulants. Apparently 'fluffers' are used too (tq above but how did you know?).
John Holmes died at quite an early age, I cannot imagine why.

They often have something called a 'stunt c**k' (I saw this in a Louis Theroux thing once) where they'll basically have a stand-in (pardon the pun!) for scenes that involve climaxing.

Long scenes? Most porn scenes don't last more than half an hour, it doesn't exactly take much to last for that long. Frankly if sex only ever lasted half an hour I'd be p*ssed off, very p*ssed off.
