With PUTTY you have to pick the Protocol
The default is Telnet be sure the click the SSH box!
That will switch you from port 23 to port 22 and should work.

You are using PuTTY correctly, but the password or user name is incorrect. Contact the operator of the SSH server to help resolve this.

are you sure you have the permission to acces teh server you are trying to access ?

I guess you think you know the pasword
I would try the following

A) Give a bad user name
B) Give a bad password

is the result any different from what you get when you enter the user\password what you think is the correct combination ?

1. Please ensure your server accepted the ssh (port 22) or telnet (port 23)
2. The error prompted you most probably is to do with you rlogin credential. So please check with your administrator to reset your login again.
3. Password policy may restrict you to enter.
4. Your account already blocked.
