Being anxious tells your body to release adrenaline, which is the 'fight or flight' hormone that gets your body ready for trouble when a lion or an aggressive person appears, or the ceiling starts cracking. It's set off by 'just thoughts in your head' because those thoughts could be 'Oh heck, a lion!' or 'Aargh! Alien invasion' - the body leaves the brain to interpret the world and decide what's dangerous. It can't tell whether the danger is a real, present thing, or just worry about the speech you have to make tomorrow. It just gets the message 'I'm afraid' and puts out the hormone that makes the body ready to deal.
How is the body got ready? Well, to be able to run or fight well, your arms and legs need plenty of nourishment, so blood gets routed to your muscles. You need to be able to see and hear well, so blood is routed to your sense organs and the brain switches attention to eyes and ears and away from thoughts other than the danger and how to deal with it.
Anything irrelevant is left with much less blood until the danger is over. And this is why you get nausea - your gut is left hanging, it suddenly has a load of food sitting there it can't digest and bam. Nausea.
Summary:
Anxious thoughts ---> more adrenalin --->blood routed away from the guts---> guts can't digest or move food along---> nausea.
Incidentally, ginger is an excellent and safe remedy for nausea.

Anxiety, when it builds up, tends to create a little 'bad feeling' in the pit of your actual stomach. So, the more you keep it inside, the more in builds up, the more you worry, the worse the feeling gets. It sometimes actually depends on how strong the feeling of anxiety is in the first place, how bad is it what you've done or what is happening or how you deal with the feeling. Can you just forget and get on with your life and learn to forgive yourself? I had problems with this because I had been upset for quite a while and I'd been on a break from school and I'd had a lot of spare time. Unfortunately, too much spare time caused my mind to start preying on things from the past that would have never had usually bothered me. But now I'm back onto my schedule and it is as fatiguing as ever.
So, really it depends on how bad you really do feel about what is happening which creates the feeling of nausea. It may just start off as a thought, but the more you focus on the thought, the more it takes over what your gut feelings and emotions are like. That feeling is your brain asking you to get whatever problem you've got off your chest because it's weighing you down and making you feel unwell.

Finally they did a belly emptying experiment. I have Gastroparesis you'll google that. I are not able to vomit given that of the reflux surgical procedure, however the nausea is horrible. They did surgical procedure and placed a gastric pacemaker in my belly. Out sufferer surgical procedure and it has helped me such a lot. Nausea comes as a result of chemicals that are released when one is anxious. Adrenaline is usually the culprit

being anxious affects your adrenalin, equalibrium, robs you of much needed sleep, possibly keeps you from getting proper nutrition, etc. In my case it was stage fright - shot my blood pressure up and messed with my balance/equilibrium which caused nausea.
