There should be a rebate form with your paperwork, use it. Fill it out and read the instructions for what type of proof you need. Usually you need the receipt and the UPC off the box, and often you can simply copy it,and sometimes you have to send the original. Usually, thats about it, those three items. Make sure you photo copy everything, the rebate formed filled out, the receipt, and the UPC label from the box the modem came in.
If you can't locate these items call customer service at AT&T and ask them whats going on with the rebate offer you had with your modem, tell them you can't locate the rebate form or any instrucitons on how to fill it out and what to send in with it. They should then resend you a new rebate form and the instructions on what you need to send in with it. Now, if you had somebody else install your modem and they took the box, you need to tell AT&T that too, and ask them to please instruct the installation person to return your box so you can send off for your rebate.
