I am filling up a form for applying to study in the States but not quite sure if Graduate Program includes not only Master program but also doctoral program.
Please help me out.

Sorry, but Martin S. is incorrect. A "graduate program" is anything beyond the bachelor's degree, so it generally includes all masters and doctoral programs. The bachelor's degree is what is called an "undergraduate" program. There is no specific degree called a "graduate".

So, the simple answer to your question is yes, this includes your Ph.D. program.

If you are in a graduate program, you have already attained a Bachelor's degree. A "Graduate Program" is either for your Master's degree or Doctorate.

Yes, most Graduate schools have a Ph.D program.
