First, simplify each of the square roots. The square root of 12 = 2*sqrt(3), sqrt(18)=3*sqrt(2), sqrt(109) = 6*sqrt(3), sqrt(72) = 6*sqrt(2).

Then collect terms to get something * sqrt(2) + somethingelse * sqrt(3).

For the second one, multiply out to get four terms, then collect terms to get something + somethingelse * sqrt(5).
