the thenur muscles of the palm side are swollen. He complains of pain (initially going up arm-that has stopped). Too painful to put ice on. His whole hand is now cold (several hours later) and the other hand is warm. Any comments, suggestions?

The fact that his hand is cold leads me to believe there may be a circulation problem. Swelling is to be expected in a hyperextension injury like that, but if ibuprofen and ice don't bring the swelling down, it should be examined by a Physician.

I would take him to the emergency room ASAP, because if there is a circulation problem, you don't want the blood supply cut off for too long.

Agreed with Lolo: Talk to a doctor. I'm not easy about that kind of injury and swelling.

Take him to the doctor.....don't take this lightly....don't wait till it's too late and the hand has to be amputated due to tissue damage.
