The fact that his hand is cold leads me to believe there may be a circulation problem. Swelling is to be expected in a hyperextension injury like that, but if ibuprofen and ice don't bring the swelling down, it should be examined by a Physician.
I would take him to the emergency room ASAP, because if there is a circulation problem, you don't want the blood supply cut off for too long.

i think it should of been broken
use a cream called "bengay" it relieves muscle tension and more.
