I am designing a directory for artists and designers and want to show a few pics of their work for those who do not have websites. Do i need there permission? And if yes how do i go about that if they approve. Are there any forms to fill out? or can we do it over the internet and print the page as proof?

I would be creating a short profile for them thst includes the name of business, founder/owner, location, services provided, a brief description of what they do, their contact info, and a link to their website if they have one. and if the didn't i would show like 3-5 images of their work. do i still need their permision? i'm basically advertising them for free

Generally as a rule it is a good idea to get written permission from an artist, however in the absence of written permission as long as you show there work and give the proper credits to the artist you shouldn't have any issues at all it is done everyday.
It does help if you give all the information you can on the artist including website etc, if they have one.
