Improper lighting- can not see well enough, so tripping, falling, and running into things occurs.
Leaking water- can cause electrocution, slipping,
Fire/heat/heaters-Things catch on fire, overheat, burn up

As far as I can tell, (I have 5 kids so I have some experience) the main cause of accidents is stupidity. People running where they should walk, people leaving stuff on the floor when it should be put away, I could go on forever...
