You can still buy toys just make sure they are either made in America or in Europe. Almost all toys have their country of origin printed on the packaging. Avoid the dollar store, because most of their products will be from China.
Alternatives depend on what you can do. For example I once took a postcard glued it to a piece of wood and then used a jigsaw to cut it up and create a custom made puzzle.
Sneakers are safe, but you may want to give a gift certificate from a store so that they can combine a few and buy what they want.
Books are always a good bet and there are a wide range of them. There are also computer games made for kids as well as education programs disguised as games.
Even a gift card at a restaurant can be a good gift, because that will make it easier for mom and dad to take them out. Gift cards for almost anything are always good gifts the only problem is that they don't require much creative thought and it is hard to get them on discount so I tend to avoid them.
Let your imagination run free. One of the biggest gifts you can give is your time. If you take the child out to lunch and a shopping trip or take them to a museum or some a go cart track or a miniature golf course; whatever they like. That is how you can really create a gift that matters.

Did you know that fully 80% of toys in the USA are made in China? The total amount that has been recalled for lead poisoning is SO MINUTE compared to the amount that is out there, as to be practically "null."
However, there are some toy stores which are expanding their "Made in the USA" toys, and I'm sure you can find some online. Have you looked at FAO Schwartz's website?
Otherwise, there may be local "artisans" who craft toys... look in the Yellow Pages. Do a websearch for "Toys made usa" and see what you get!

I have an alternative. I'm going to make the toys myself. Right now I'm working on a Nintendo Wii that's made out cherry wood.

Few (trusty) toys: Just Care Bears, an mp3 player or two, clothes, hoodies, some CDs (burned and bought) and that's about it!

That's a good description. Now we know why so many children suffer from lead poisoning and the lifelong effects. My daughter got it and has been learning disabled, ADHD and slow learner because of it. It is serious so thanks for the mention right before Christmas.

I know this is very scary BUT you shouldn't get too paranoid (for want of a much better word) about it. Life is dangerous. Kids are exposed to many varied and dangerous substances, situations and infections from birth. You can do your best but you can't completely protect them from living.
