Maybe some framed enlargements of old family photos?
The poem suggestion is brilliant. If you want any help writing it, email me and maybe I could help.
Good luck!

I like a lot of the ideas above: photo frame, spending time with her, etc.
I'd like to add a few ideas to the list:
Flameless candles--they give off a nice scent, but are battery operated so there's no danger from a real flame. You can get these at Target or Brookstone, among other places.
A throw blanket: It gets cold in nursing homes, and a nice warm fleece blanket she can wear all the time would be comfy.

Something edible is always a good idea. Think dried fruits like a nicely presented box of dried apricots. Or a gift box of fresh citrus fruits. Steer clear of any sweets like cookies or cakes. Nuts might not go down well either since they can be hard to chew at that age.

Make her a nice poem about her life.
She would really appreciate it. Have her hang it
up in her room so she can remember how much you\
love her, even when your not visiting her in her nursing home.
The best gifts come from the heart.
Happy Holidays

Does she have a nice robe and slippers? Fancy sweater? Pics and any personal writings are always treasured. Make a date to take her shopping or to a park or museum.
