How to do a written application for a job?

Im 17 and applying for my first ever job as an apprentiship office worker and they train you.
They are asking for a written application and i have no idea where to start. Anyone any suggestions? Or know any sites that can help?

They will provide it. DO NOT PRODUCE YOUR OWN. If they want a resume, which is quite different, pick up a book at the library.
