I held the carton as I usually would and I ran my hand down the shaft of the carton placing it at the usual spot, and then I tilted the shaft and the milk came spurting too quickly out of the spout.

You may have slightly squeezed the carton at the same moment that you began to pour and that would result in a sudden rush of milk, or the milk may have been partially frozen. This would cause the pressure inside the carton to increase so that when it was opened the sudden release of this pressure would cause the spill.
