you said "HPV again"? So she has a history of HPV?
If so, it is possible she is having a flare up and I would recommend her getting it treated to reduce the risk of her spreading it to you.
If it is just a pimple, it should clear up on its own

Pap smears help detect cellular changes on the cervix, which are most often caused by HPV. It has nothing to do with anything that is occurring ON her vagina.
If it's not just a pimple (folliculitus is much more common in the genital area, and highly resembles a "whitehead" pimple), or irritation from razor burn, it's most likely herpes.
When someone is having an outbreak with herpes, one of the stages of the outbreak involves lesions that can resemble "whitehead" pimples.
When someone is infected with the strain of HPV that causes genital warts, the lesions look like, well, warts. They are often cauliflower-like in appearance, and come in all shapes and sizes.
The best thing to do is have her make another appointment with her gynecologist, or with a general practitioner. This would need to be done ASAP so that a sample of the lesion could be taken. This is never pleasant - it can involve just a scraping of the area, or a biopsy. But it's better to know than to assume.

If it's just one pimple then it COULD be just a bacterial thing on the skin. Keep it clean and if it doesn't clear up then talk with a doctor.

could be an std pap smears dont test for all stds tell her to see the doctor

does she shave? sometimes its just from razor burn, or if you live in a humid place and she sweats it could have just formed there...
