they can break. They can slip off inside of the woman. But this is not why it is 99 percent effective. If these things happen, than that means it is 0 chance effective
The 1 percent is because they have pores and sometimes a little guy can get through. That is why they say 99 percent effective IF used properly

What causes condoms to break?
Condoms too old — Modern condom wrappers have a date after which the condom should not be used.
Improper storage — Heat damages latex condoms, so they should not be kept in a hot place, such as a car glove compartment or wallet.
Not enough lubrication — Additional lubrication is always needed for rectal sex. It might also be needed for vaginal sex. The lubricant should be water-soluble, such as KY jelly.
The wrong kind of lubricant — Lubricants that contain oil—such as Vaseline, baby oils, and vegetable oils—should not be used with latex condoms since they weaken the rubber.
Condom too small —Try a larger size.
Partner too tight — Use an extra strength condom and more lubricant.
What causes condoms to come off during sex?
Condom too large — Try a snug condom.
Loss of erection — Remove your penis, holding on to the rolled edge of the condom, as soon as you begin to lose your erection.
Why aren’t condoms 100 percent effective?
The most common reason for condom failure is not using it properly. The most common reason for not using a condom is not having one. BE PREPARED. With careful use, condoms give excellent protection.

Well, there is a chance that they could break or they could even come off while having sex. I learned back when I was in school that a condom could have a tiny hole in it and no one would even know...Those sperm are VERY small and they are fast...So that is partly why they aren't 100% effective....Chances are slim that a girl could get pregnant if her and her man use them correctly.

In rare cases they pop if enough sperm ejaculates into the end. Also sometimes fricten will pull them off the erect penis as it goes in and out of the smaller vagina. Some horrible teens have been caught going into stores and inserting pins into the condoms through the package so sperm will leak out into the girl and make her get pregnant. You can get pregnant if the boy ejaculates sperm just on the crease of the vagina lips, cuz it will seem pnto the hole and the sperm will swim in and impregnate the girl.

They can be put on wrong, they can fall off, they can break or have a tear in them.
If you don't remove your penis immediately after ejaculation while holding the condom, the condom can fall off.

They can break without you knowing, many people don't put them on properly, they can be torn by fingernails etc before they're put on, they can slip off during sex...the list goes on.
