You can only be removed as a co-signer with the agreement of the other party to the contract. Don't expect to receive agreement on that, since the person for whom you co-signed would not have needed a co-signer without a reason.

it depends on the contract, but in general co-signed on a note like car loan only if the person can refinance, if you co-sign on a apartment then at end of the lease request to be remove

Yes, if the lender/ creditor will give you a "release of liability" letter. Usually they will not, however, you may want to ask. Another way is to have the primary borrower refinance the original loan and take you off.
