Team colors are up to the team's front office or ownership to decide. Change is not uncommon these days, particularly when the marketing research shows that merchandise sales are going flat (market saturated, consumers satisfied with what they already have) -- then a change of colors or design can spur new sales among fans who want to stay current. However, this tends to happen mostly with the younger teams; older, original teams benefit from having "classic" (essentially older than living memory) regalia that sells well consistently.
Here's an example: the Rangers went to a primary-red design when they moved into The Ballpark in 1994, dumping the old primary-blue. After Hicks bought the team, he changed back to a blue scheme. The general, league-mandated scheme is for the home team to employ a lot of white, and the visitors to show gray (or "drab"). Also, again in general, the home team unis will have the team name (little point in showing the city, as that is where they are), and the visiting team displays their home city (though not every team does this). Its all in the Marketing, Most teams though wear the mascots name at home, and the actual town they play in on the road. This helps newer fans watching their team playing at home, to know where the visiting team is from. Meanwhile when the team plays at home, obviously the fans Know where their team is from!!! But not all teams do this and I think in the case of the Angels its because they have to call themselves the Los Angeles Angels OF Anaheim!!! and they play in Anaheim, So they don't want people to think they play there!! This stems from a law suit between L.A. and the team, which the team lost!! Also any owner of any sports team can throw whatever uniform they want to out there, its just like McDonald's deciding to make their employees wear a tie, they can do whatever they want!!! Except road uniforms typically have to be white, but the home team wants again decides which uniforms the opposing team wears, White or Collards!!

2. Away team usually wears some form of gray
3. It is completely each team's prefrence on what to put on the jersey, like the example you gave of the angels is true, but like my brewers have the exact same uniform that says brewers, it is just that one is white and one is gray for home and road games.

Just to make a small correction on the 1st guy's answer. Road teams generally wear darker colors and the home team wears white, but it can vary at some points.
