This will give you some idea of states that ban smoking in restaurants, its not an exact science.

Dining: These states ban smoking in restaurants and bars: California, Delaware, New York, Connecticut, Maine, Massachusetts, New Jersey (casinos exempted), Rhode Island, Vermont and Washington. Montana banned smoking in restaurants - bars exempt till 2009. South Dakota had a ban - status unsure. In November 2006, Ohio and Nevada banned smoking in most public places; Arizona passed a similar ban, which takes effect May 2007. In January 2006 the Territory of Puerto Rico banned smoking in the workplace, including bars and casinos. Illinois will be as of the first of January. It is a smoking ban in all public places.

Ohio does and after spending a day in Michigan (where they don't) I sure do like the law!

Its more of a city ordinance thing in my experience. Various cities have ordinances against it. Not states in particular.

I think it is more city ordinance that a state thing. Nebraska has some cities that do that..

I think all of Canada or most of it has banned it in resaurants.
