I have never heard it used except in a negative way.

Egotistical pretty much means "self-centered" which would be considered a negative trait. Probably the positive side of that would be a word like "confident" or "self-assured."

I would never want to be labeled as egotistical. It's a very negative trait. It's meaning is self absorbing and self involved. It's a very bad character trait.

It's normally used to refer to someone in a less than flattering and negative way.
