WOw! I'm hesitant to answer this question only because I believe that 1) beauty is in the eyes of the beholder 2) physical conditions or injuries could make up for a person being what 'you'd ' call hideously ugly. 3) I think of the fable "Beauty and the Beast" and always want to give people the benefit of the doubt 4) I believe in the Word of God that says we might be entertaining angels or Jesus himself could come in a form unlike we expect and would we be so quick to turn him or angels away? Hideously ugly can originate from the inside. A person whose life is filled with hatred and remorse, no joy or love or peace in his heart or spirit can exude great ugliness. Someone who is born with features that we are unaccustomed to seeing might bring about curiosity, but they don't have to be hideous. Example, I know a boy who was born with a deformity from his nose to lips. They were infused and yes he did look different, but I could never say hideous. He was sweet inside and out. Eventually, the community helped to pay for the surgery which separated his lip and nose and today he is 'normal looking' and still has the personality of a sweet heart.

A negative or narcisstic personality or someone who is incredibly egotistical. Perhaps being a liar and unfaithful partner. Many negative things to a personality.

a strange bone structure maybe, serious amounts of acne, bad hygiene (like bad breath, greasy hair, B.O)
but personalities can make someone ugly aswell.

well, unnatractiveness is linked to bad bone structure, lack of symmetry in the face and unproportional features like if your face is shaped oddly of your eyes are too small or if your jaw is too big for your face

eyes too small that irises are not shown, eyes too large that are roswell alien-like, large, wide nose, big full mouths, acne and other skin conditions, black or yellow teeth (my grandpa had that teeth because of smoking), horrendlously fat or anorexic
