Not understanding how life works. Becoming frustrated because the apparent results don't seem to fit the effort. Interestingly some people do not give up and many people actually realize their dreams.
Bottom line, you must learn as you go along. If you do not, you will eventually get frustrated and give up hope(your dreams). I suppose one would not consider themselves a failure if they never made an attempt in the first place.

Lack of success
No encouragement or getting ridiculed for your dream
Lack of ambition
Financial considerations
Finding out that the reality of achieving that goal is different than previously thought
Changing priorities in life
Loss of interest

I belive its many things, like a lack of planning, distractions, compromising too much, and not having a rational plan to meet the dream. Sometimes dreams are short-term, and you find out you really did not want it bad enough in the first place.

Stark reality that stares one in the face, the naked truth that the dream had to remain a dream for the majority of us. It is only fortunate few whose dreams come true and then they can ascribe it to their hard work and perseverance. But the truth remains that for every dream that comes true there are a million that are shattered every day. Only a chosen few by God can make their dreams come true.
