Do you mean as an employment option or as a patient?
If you mean being a patient, the only thing that you would probably need would be an insurance card if you have one....
Also you would need the name of an emergency contact person when you fill out the form, in case they have to call someone.
If you mean as an employment option, then you would need your CPR card, your ID, your current license, and a pen to fill out the forms..

Oh, if you are a patient, then you would also need the name and number of your current doctor and any information you could provide as far as previous illnesses, and/or surgeries. It would also be a good idea to take a list of your medications with you if there are a lot of them, and your allergies..all of this would help the ER help you.
