In general, obtaining travel insurance (for just this sort of thing) is the responsibility of the traveler. If the individual did not obtain the incurance in advance (or it was insufficient for the costs incurred), the balance is payable by the individual.

Medical treatment is NOT free for visitors to any country. It is a "pay-as-you-go" process. Although the medical facilities cannot (usually) refuse to treat someone, they DO have the rights for financial recompense for their efforts.

At the bottom line, they can sue the individual for the amount owed (usually in a court in the medical facilities' country, where the laws are in favor of the facility/practitioner), but it will apply to the individual regardless of their country of origin. The hard part may be in recapturing the owed monies ... but with such a suit against an individual, it will be very difficult (if not impossible) to re-enter the country where the suit was issued until the repayment is complete.

EDIT: You didn't read the whole answer I gave, if you're asking if he can be sued in his own country. He could be, but it's more to the advantage of the medical facility to sue in THEIR country. Then the respective governments and collection agencies take over, and track him down in his nation to make him pay the debt. If they haven't caught him by the time he tries to return to the country where the debt was incurred, he will not be allowed to enter until the debt (including accrued interest) is paid in full.
