Is there a good way to convert CMYK colours to Pantone colours?

According to Wikipedia, the Pantone colors appear to be a superset of the CMYK colors. For this reason, all CMYK colors should transform into a Pantone color. Since the details are closed though, I can't provide any function to do this.

That is a Photoshop option. Look at Navigator that usually docks in upper right hand corner.
