They mess up the homone cycle of the body so that the uterus, instead of getting ready to receive an egg, thinks it already has one and won't accept another. The long term pills keep it in this condition for longer than 21 days. The demonstration of this is the period that occurs at the end of the active pills. I think Alpla has the best answer. While some birth control pills affect ovulation, others make the uterine lining less receptive to sperm. In order for conception to take place, the sperm must fertilize the egg which then attaches to the uterine wall where it develops into the embryo. Some birth control pills inhibit this process.

Most birth control pills are estrogen-based and fool the body into thinking it's pregnant already. By doing so your body will not produce ovaries during your regular 28-day cycle. You will only take the pills for 28 days, and then you will abstain from them for 4 days or until menstruation ceases for that cycle.

The common theory is that they interrupt the play between pitutary gland and ovaries that trigger ovulation. Plus they modify the mucus of the cervix of uterus so it's less friendly to sperm.
Now the real truth is nobody knows for sure how and why they work.

They make you body produce too many (or too little) hormones to get pregnant. Some times your body will not release eggs until hormone levels are sufficient. (Thus some people have twins or triplets when they stop taking birth control)

she asked how do they work not what do they do...... what is best is that you talk to and OG-GYN and have them best explain it and best choose the right one for you! womens bodies are created different so what may work for your friend may not work for you! Personally i have tried Yasmin and they work fine and i didnt see any changes in my body, except for the fact that i never got pregnant, and i dont want to sound like one of those commmercials but it doesnt protect you from STD/AIDS or anything else so please be carefull
