What forces, over what period of time, contributed to its formation?

The latest I've heard is that there is a super volcano under Yellowstone. Magma comes close to the surface there and heats water and creates steam. At a certain pressure the steam is released.

An underground hot spring formed old faithful and it erupts every 15 minutes and is also powered by the Yellowstone Super-volcano in NW Wyoming.

A huge pocket of magma that is building under old faith-full, is heating the ground water above the pocket which causes old faithful to blow periodically.
