I thought Wolf Creek was pretty scary as it was based on a true story but the original Halloween is enough to make you shiver under the covers.The Saw movies are good too.

The absolute creepiest movie I've ever seen was the one with Mel Gibson and the crop circles, "Signs". It was directed by M. Night Shyamalan. This was classic horror, reminiscent of Alfred Hitchcock. You did not actually see a monster until WELL into the movie, but you did catch little glimpses of them. It was not a blood & guts horror movie, but a lot of suspense and sharp turns. It gives me shivers just thinking about it! I had bad dreams for days after watching that in the theater.

This is a really unconventional response because it is not a horror film in any sense of the word. The film is called 'Shame' and it is Australian, made in the 80's. The reason I found it so scarey was because it is totally naturalistic and plausible, and if you empathise with the young female characters then you feel the fear of rape too.
But like I said - this is not a conventional response - I am just going on what I felt.
28 days later is pretty scarey too, but do not bother with the sequel

Perhaps this does not entirely meet what you are in search of, but an old French short film titled La Jetée has always been quite interesting to me. It is done entirely in gray scale photos with a voice over, and the plot is intriguing and disturbing, without ever being, as you say, stupid, gruesome, or macabre.
It may be purchased on Amazon for a reasonable price, if you desire the legal path.

If you want scary.... The Excorsist still gives me nightmares because it's real. Just happened to a boy.
The ring is scary but, not that scary. I saw The Decent and I screamed a couple of times. That scared the hell out of me.
The Blare Witch Project.
Frailty was on the top 25 list.

The scariest movies aen't the ones with lots of murder, blood and gore. They are the psycological ones that get into your head and really scare you. Have you ever seen Misery or Whatever happened to Baby Jane? Scary!

I loved exorcism of Emily rose too. and I am in somewhat the same predicament...one movie I LOVED was disturbia, it was more thrilling than horror but I haven't been that into a movie for a LONG time, I loved it.

Repulsion which was made in 1965. It starred Catherine Deneuve & was directed by Roman Polanski. I have seen a ton of movies & this one will keep you awake.
