When you see that it costs $100 a square foot to build a home (or however much it is in your area), does that include kitchen appliances like dishwasher, stove, etc. and bathroom fixtures like toilets, tubs, etc?

This is a contractural issue. The plumbing (sinks, tubs, toilets) are included. A dishwasher may or may not be included. Stove, frig and other appliances are not usually included at the building price but will be installed if you have them in the contract. This would be an extra cost though for purchasing the appliances.

For most home construction, the price per square foot will include most appliances. It always includes things like the toilets, sinks, bath fixtures, etc. For the kitchen, it will include an oven of some type and usually a dishwasher (but not always). However, things like a refrigerator or washer/dryer are usually not included unless a builder is trying to offer them as incentives. I've seen all sorts from builders including big screen TVs. Also, some houses will include a microwave as well.

Look at it this way, if it's built-in it will be included.
